Key Areas of Care & Support

Personal Care

Our carers can assist you with all of your personal care needs, including:

  • Maintaining your personal hygiene

  • Maintaining your oral hygiene

  • Assiting you to get dressed or change your clothes

  • Continence care including specialist support

  • Hair washing and grooming (shaving)

Medication

Our carers can support you with your medication needs, including:

  • Ordering your medication

  • Picking your medication up from your chosen pharmacy and bringing to you

  • Checking your medication stock

  • Prompting you to take your medication, for example from a dosset box

  • Administering your medication, whether they are in a blister pack or original packaging

  • Applying creams, medication patches, eye drops, ear drop

Nutrition & Hydration

  • Preparing a meal of your choice

  • Heating up or cooking your meals

  • Serving your meals to you

  • Making you a drink of your choice/leaving you drinks out for the day

  • Monitoring your food and fluid intake daily.

Key areas of care and support

How we plan your care

We will start by arranging to meet with you to complete an assessment of the amount of care and support you require. You can invite anyone who is important to you to be present during the assessment. This is our opportunity to really get to know you well.

All assessments are completed by either the Manager, Deputy Manager or Care Coordinator. This assessment is best completed in your own home.

When the assessment is complete, we will begin developing your personalised care plan and risk assessments. You will be able to review this throughout and make any changes that you think are necessary.

We use electronic care-planning systems to keep all of your personal information safe and secure. This information can only be accessed by authorised colleagues within Palm2Palm Care.

When all of your needs and preferences are confirmed, we will aim to match you with the right carer based on their skills, personality and experiences.

We will work together with you to ensure that you receive the right amount of care at the right time(s), whilst ensuring constant review to identify any positive changes we can make.

    I have worked in social care for many years so have first-hand experience of the quality of care and what that looks like. I have to say on behalf of Mum, myself, and my brother Mum's care cannot be faltered in any way. From the initial assessment of needs meeting to the very last day of support for Mum. 10/10!

    Going the extra mile cannot always be afforded care providers and individuals but most definitely can be used as a way of positive feedback in our experience.
    Being on the end of the telephone day and night was such a comfort giving us peace of mind and a friendly empathetic ear when needed. Second to none.

    You should be very proud of the way you and the staff have conducted themselves as they are a credit not only to the company but the care sector.

    To anyone needing home care, Palm2Palm is the company to use. I have had help for my mother in the past but nothing compares to the lovely friendly carers, who always greet Mum with a great big smile. They are gentle, attentive, reliable and very professional. They also take the time to listen encourage and compliment her, which she loves. Nothing is too much for them. I’d like to say a big thank you to every member of the team, and keep up the good work! 10/10.

    My mother's life has dramatically improved since her carer came to her house. The carer treats my mother delicately and with compassionate care. She has transformed my mother's quality of life. She is patient, understanding, and ensures my mother's needs are met with dignity and care. The carer's attention never waivers and her attention to detail is always 100%. My mother describes her carer as an angel that she has been blessed with.

    I would like to say I am very happy and so is my brother who lives with my mum with the quality of care my mum receives, my mum can be very challenging at times and the staff always remain both friendly and professional and do provide an excellent service and I know my mum is being looked after and treated with respect from the care staff at all times which gives us peace of mind.

    Palm2Palm is an exceptional company and provides exceptional services in caring. The carers/staff are fantastic and go beyond their duties to make their client feel special, and helpful to the family too. My mother was under their care for almost two years and we wouldn't have it otherwise. They give excellent service and support. Unfortunately, I lost my mother in December 2023 but having a father who will need caring too in the near future we will not hesitate to use Palm2Palm again. All we can say is a big thank you to them. They made my mum's difficult life feel easy. She always looked forward to seeing the carers - they were her friends and shared any problems that she had in her dementia mind. She was treated like a queen.

    As for me, I was at ease knowing that mum was cared exceptionally well and I did not have to be with her at all times during the day which has given me the freedom to work and carry on with other daily chores. A big thank you from me.
